Lodi, CA 95240 SR0065653 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the permit application for upgrading dispensers to Dresser Wayne Ovation 3 + 1 blender <br /> dispensers, installing Bravo Adaptor frames, upgrading existing monitoring panel with DIM <br /> module with "Cold Start", converting existing 91 (8K) tank to diesel, and converting existing 89 <br /> (10K) tank to 91 product on the underground storage tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. A copy of the alarm <br /> history shall be printed prior to performing the "cold start" of the Veeder Root system <br /> and made available at the time of EHD inspection. <br /> b. Bravo conversion frame inspection. <br /> c. Leak detector test: Leak detector must be tested to confirm that the device can <br /> detect a leak of 3 gallons per hour at 10 psi. <br /> d. Verify that the diesel vapor piping and bucket have been capped off. <br /> Please be aware that the facility cannot operate the UST prior to the EHD <br /> inspection and approval. <br />
