I inadvertently included violation 203 <br /> and removed this violation in the amended inspection report. Please complete and submit a copy of the Return to <br /> Compliance Certification form to the Environmental Health Department with a statement documenting the corrective <br /> actions that have been or will be taken for each violation (excluding violation 203), and any supporting paperwork, by <br /> January 19, 2017. <br /> Please contact me if you have any questions concerning the inspection. <br /> 1 <br />
