Waterloo Rd. <br />Stockton,Ca. 95205 OCT 1 2008 <br />To:Aris Cacapit ENVIRONMENT HEALTH <br />San Joaquin County Environmental Health PERMIT/SERVICES <br />Please find enclosed Return To Compliance Certification, along with Certification of <br />financial Responsibility, and statement of repairs per Inspection during Annual <br />Monitoring Certification dated 9/08/2008. <br />#4 Certification Of Financial Responsibility <br /># I I Per the last D./O. Inspection all sensors were in the proper place, Lowest place <br />available, This station "Waterloo Liquors" along with several other sites that I inspect <br />have had sensors moved around and I have been checking each site carefully, There has <br />been several contractors in the dispensers looking for ways to pull wires for the new <br />enhanced vapor that is going to be required by April of 2009 especially the sites with <br />bravo pans, I believe that is how this sensor got moved, as I have had this problem at <br />several other sites and am trying to keep an eye on this problem. <br />#12 Elite 4 contractors removed and replaced the 87 leak detector that failed, no paper <br />work or bill has been received by Mr. Chea at Waterloo Liquors for this work as of this <br />time to provide to you. <br />#18 Same as # 12, Elite 4 has done the work, to replace the 91 spill bucket that failed, <br />but we have no paper work or bill from them to provide as of right now. <br />Will be glad to provide any paper work you need from elite 4 when we receive it. <br />Thank You, Dan McILrath D./O.
