4. All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing <br />organization. <br />5. All monitoring equipment must be approved and listed in the State Water Resources <br />Control Board's Local Guidance Letter 113:"List of Leak Detection Equipment and <br />Methods for Underground Storage Tanks." <br />6. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) for the diesel tank no later than 30 days after work is completed. <br />7. This permit expires 90 days from the approval date. <br />8. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
