All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />and <br />d. Each person installing or testing any component must hold a certificate of training <br />from the manufacturer of that component. <br />4. All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing <br />organization. <br />5. All monitoring equipment must be approved and listed in the State Water Resources <br />Control Board's Local Guidance Letter 113:"List of Leak Detection Equipment and <br />Methods for Underground Storage Tanks." <br />6. A tank integrity test must be performed by a third party certified tank tester and <br />results received by this office within 7 days of completion of work. <br />7. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) for the 91 tank no later than 30 days after work is completed. <br />8. Complete and submit an updated monitoring plan to this office for approval no later <br />than 30 days after the work is completed. <br />9. This permit expires 90 days from the approval date. <br />10. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
