Waterloo, Stockton <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II or MINOR -Notice to Com <br />21. (Repeat) Observed water in 87 grade UST sump. <br />* Facility shall contain a means of monitoring for water intrusion and for removing the water by the <br />owner or operator, and provide a means of analyzing the removed water for hazardous substance <br />contamination and a means of disposal. <br />- Immediately remove water from the above sump, and make a hazardous waste (HW) determination <br />of the removed water. This violation has been corrected on site. The removed water has been placed in <br />5 -gallon HW containers on site. <br />23d. (Repeat) Designated UST Operator (DO) employee training log not available for inspection. <br />* Facility shall retain a copy of the current DO employee training log. Facility employee shall be trined <br />by DO every twelve months. <br />- Submit a copy of the current DO employee training log to EHD by July 30, 2010, and continue this <br />practice. <br />23a. Failed to inform EHD change of DO within 30 days. <br />* Owner of UST system shall inform EHD any change of DO no latter than 30 days after the change. <br />- Begin and continue this practice in future. <br />23c. A copy of the June 2009 to January 2010 DO monthly inspection reports (DO reports) were not <br />available on site for inspection. <br />* Facility shall retain a copy of the last twelve months of DO reports available for inspection on site. <br />- Submit a copy of the missing DO reports to EHD by July 30, 2010. <br />24,26. Monitoring equipment certification (MC) and the spill containment tests were not performed <br />annually. The last MC was conducted in September 8, 2010. The last 91 grade and Diesel spill <br />containment test was conducted on September 8, 2010, and the 87 grade containment test was <br />conducted on October 1, 2008. <br />* MC certification and spill containment testing shall be tested annually. <br />- Immediately have a properly licensed contractor tested the line leak detectors under EHD inspection. <br />This violation has been corrected on site. <br />NOTE <br />-A revised version of this page correcting Item 24,26 was mailed to generator on July 20, 2010. <br />-Submit a copy of the Return to Compliance Certification (provided), a statement how the above violation <br />were corrected, and any evidence of correction to EHD by July 30, 2010. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TINE AT EHD'S CURRENT HOURLY RATE. <br />EHD Ins ctor: Vv <br />Received By: <br />Title: <br />-V r U v cl— <br />EHD 23-02-003 Rev 06/01/10 CONTINUATION FORM <br />
