Waterloo Rd., Stockton CA <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the revised maps and comment responses by Franzen Hill submitted to EHD on June 17, 2010 <br />in support of the permit application SR0057217 to install a VST Phase 11 Hirt Thermal Oxidizer <br />System without In -Station Diagnostics (ISD) for EVR upgrade at the above noted address. <br />SR0057217 was originally submitted to EHD on May 1, 2009, and had since been revised and <br />submitted to EHD on March 4, 2010, March 25, 2010, and June 17, 2010. It has come to EHD's <br />attention on March 1, 2010 that the proposed work has already been implemented at the above <br />noted address in December 2009 without prior approval from EHD. The permit has been <br />approved retroactively with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. <br />b. 24-hour hydrostatic test (or equivalent) on 91 grade STP sump. <br />c. Secondary containment testing on the fuel piping and the 91 grade underground <br />storage tank (UST). <br />X41,F41.4ho-E-141, <br />.nspection and approval. <br />4. Since concrete is broken near the piping and the tank, a secondary containment test <br />must be performed on the fuel piping, as well as the 91 grade tank once concrete is <br />poured in accordance with the manufacturer's guidelines. EHD inspector must be on- <br />site to witness this test. A 48-hour notice is likewise required for inspection <br />appointments. <br />
