A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced; and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />7. Contact Jeffrey Wong (468-0335) to arrange the following inspection. A 48-hour notice <br />is required: <br />a. The EHD must witness the functionality of the monitoring system to ensure <br />that monitoring has been restored to meet the requirements of California <br />Code of Regulations Title 23 and the Health & Safety Code. All monitoring <br />system sensors must be tested. <br />8. An "Underground Storage Tank Certification of Installation/Modification" form (Form <br />C) shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(4)] <br />before this upgrade will be considered complete by EHD. <br />9. Permit expires 180 days from date of approval.
