Waterloo Rd. <br />Stockton, CA 95205 <br />Phone:►' 468-3420 <br />(209) 464-0138 <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, RENS, RDI <br />Kasey Foley, REHS <br />Linda Turkatte, REHS <br />MAM <br />Flo I i •. •. <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the permit application addendum to break concrete on north (lower side) of Dispenser #1/2 and <br />to excavate under 91 -octane side to remove existing repair and clean top and bottom of UDC, <br />install Diversified Shallow Dispenser Pan Repair, and install stainless steel bolts and nuts to <br />secure entry to UDC on the underground storage tank (UST) system at the above noted <br />address. Applicant stated no breaking of primary and secondary lines or drilling into the UDC. <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. The proposed equipment/components must be installed in accordance to manufacturer's <br />instructions and third -party approved. <br />4. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. 24-hour hydrostatic test (or equivalent) on Dispenser #1/2 UDC sump. <br />b. Secondary containment testing on all fuel piping. <br />c. Corrosion protection: Verify corrosion protection for the underside of the UDC where <br />stainless steel bolts and nuts are installed to secure entry to UDC prior to backfilling. <br />.. <br />approval.inspection and <br />5. If concrete will be broken near the UST or piping, a secondary containment test must be <br />performed on the UST or piping once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />
