Waterloo Rd. <br />Stockton, CA 95205 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace boots on UDC #1/2 and #3/4 using Diversified Shallow <br />Dispenser Pan Repair Kit on the underground storage tank (UST) system at the above noted <br />address. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. The proposed equipment/components must be installed in accordance to manufacturer's <br />instructions and third -party approved. <br />4. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Secondary containment testing on both 87 octane and 91 octane fuel piping. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />5. If concrete will be broken near the UST or piping, a secondary containment test must'be <br />
