On March 25, 2010, Mr. Warren submitted the revised site maps, which corrected the <br />location of the USTs, but did not address any of the other discrepancies in the previous <br />submittal. The site maps appeared to be revised by redrawing the canopy posts on the <br />other side of the dispensers, and the conduit trench line was drawn angled to this new <br />location. The site maps also indicated that a new vent line was installed from the 91 - <br />octane sump to the other canopy post, with a 45 -degree angle. The existing UST piping <br />was not shown, and there was no information whether the former vent line was modified, <br />removed or capped in place. Attachment 37 is a copy of the map showing the affected <br />areas. <br />On March 29, 2010, Mr. Wong drove by the facility and found that the conduit trench <br />went straight from the building to the canopy post, and the trench for the new vent line <br />made a 90 -degree angle, both contradicting what was indicated on the site maps. The <br />existing above grade gasoline vent lines were not removed (Attachment 38). <br />On April 5, 2010, Mr. Wong called and left a message for Ms. Lopez regarding the <br />discrepancies found in the site maps that Mr. Warren submitted and asked that she call <br />him back. <br />On April 6, 2010, Mr. Wong spoke to Mr. Warren and explained the additional <br />information that was needed before the permit application could be approved. Mr. <br />Warren asked that Mr. Wong send him the information by email. Mr. Wong sent the <br />information via email to Mr. Warren and Ms. Lopez and mailed a hard copy to Mr. Chea <br />(Attachment 39). <br />On May 5, 2010, Mr. Wong called Mr. Warren asking for the information required to <br />approve permit application SR0057217. Mr. Warren stated that he went to the facility, <br />took measurements, and gave the information to their engineer to draw up the site map. <br />Mr. Warren stated that he would have the site maps delivered to the EHD office by May <br />13, 2010. Mr. Wong followed up with an email to Mr. Warren and mailed a hard copy to <br />Mr. Chea (Attachment 40). <br />On May 13, 2010, a revised site map was submitted. Upon review of the map, it was <br />found that the diagrams still lacked detail showing how the new vent line was connected <br />to the tank, and if the old vent lines were still connected or not. "Existing vent lines to <br />remain connected. Exact location unknown." was handwritten on the map. Mr. Wong <br />called and explained the deficiencies to Ms. Lopez. Attachment 41 is a copy of the map <br />showing the affected areas. <br />On May 19, 2010, Mr. Wong sent an email to Ms. Lopez explaining the information <br />needed to approve permit application SR0057217 (Attachment 42). Ms. Lopez <br />responded the follow day stating they were working on providing the requested <br />information (Attachment 43). <br />On June 17, 2010, Mr. Wong reviewed the facility's file and found that the annual <br />monitoring system certification, leak detector testing, and spill container testing were last <br />performed on September 8, 2008, and were all nine months past due. Mr. Wong met <br />Mr. Chea on site and explained that the EHD would accept the results from the October <br />7, 2009, cold start inspection as the annual monitoring system certification, but the <br />testing of the leak detectors and spill containers were still past due. <br />R <br />
