The designated operator failed to check and note the correct test dates for the secondary containment testing, monitoring system certification and spill container testing on the August 2014 to June 2015 designated operator monthly inspection report. The following had the incorrect test dates listed: 1) monitoring system certification was listed as 7/20/14 for the monthly reports of August 2014 to June 2015 2) secondary containment test date was listed as 7/30/12 for the monthly reports of August 2014 to June 2015 3) spill container test date was listed as 1/23/12 for monthly reports of July 2014 to November 2014 During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to document all the alarms from the attached alarm history on the August 2014 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: fuel alarm for L1, L2, L3, L4, L5, L6, and L7 on July 18, 2014. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). Liquid was observed in the 1 cup of liquid in 87 turbine sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The designated operator removed 1 cup of liquid from the 87 turbine sump. Ensure that all sumps and annular spaces are maintained free of liquid. This is a minor violation. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector twice, retested it, and it passed. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by August 23, 2015.
