The following are <br />Minor Violations - Notice to Comply: 201, 204, 311, 999 <br />201. The following fuel alarms were entered on the wrong date on the daily UST Monitoring System <br />Log, and/or entered as No Alarm: 8/10/2012 L6 and L7 fuel alarms, 12/2/2012 L3 fuel alarm, <br />12/24/2012 L6 fuel alarm, 3/13/2013 L7 fuel alarm. Monitoring records, including maintenance logs of <br />monitoring equipment shall be recorded accurately and kept on site for at least 3 years. Immediately <br />begin maintain all maintenance and monitoring records on site per this section. <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />4/19/13, 3/6/13, 1/3/13, 12/11/12, and 8/10/12 designated operator monthly inspection reports, and the <br />check boxes of Item 4 (each alarm has been responded to appropriately) and Items 5 and 5a (Sensors <br />have not alarmed in the past month, and list of all tank -top sumps alarms) were marked erroneously on <br />these same reports with the exception of the 3/6/13 report which referred the alarm documentation and <br />response to the corresponding Monitoring Log (see Item 201 above). The missing alarms include: <br />8/10/2012 L6 and L7 fuel alarms, 12/2/2012 L3 fuel alarm, 12/24/2012 L6 fuel alarm, and 3/13/2013 L7 <br />fuel alarm. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />311. The 87 -octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detector, retested it, and it passed. <br />999. HSC 25404 (e)(4). The UST portion of the CERS electronic submittal requirement has not been <br />met. Submit the required UST information on the CERS website by 8/30/2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />T <br />
