Jacktone Rd., Ripon CA <br />a <br />A recent review of the underground storage tank file for the above referenced facility indicated this office has <br />-not received this years testing and paperwork requirements. <br />The following information is overdue: <br />_ CCR 2637(b) Annual monitoring certification - Past due on August 21, 2004. <br />_ CCR 2630(d) Annual line leak detector test - A permit has been issued by our department, still need <br />to schedule an inspection. <br />Note: <br />- CCR 2637(a) 36 -month secondary containment test is due on or before March 24, 2005. <br />Most forms may be accessed from our web site address, ivtivw.geocities.contlunitiii <br />In addition, this office must be notified of any test date and time within 48 hours of performing the test. <br />Furthermore, results from the testing must be sent to this office within 30 days of the test completion. Under <br />CCR 2637(b)(4) it is the owner/operators responsibility to submit test results to this office.
