Ir W <br /> Dennis Catanyag [EH] <br /> From: Dan Crawford [dan.crawford@flyingj.com] <br /> Sent: Thursday, July 28, 2005 11:50 AM <br /> To: Dennis Catanyag [EH] <br /> Cc: gm.ripon@flyingj.com; 'dfm.cwell'; todd.veibell@flyingj.com; Paul.Siler@flyingj.com; <br /> Rusty.Mace@flyingj.com <br /> Subject: Ripon, CA-Status of SB989 Testing and Repair Work at Flying J Travel Plaza <br /> Importance: High <br /> Dear Mr. Cantayag, <br /> In order to keep you abreast of the SB989 activities being completed at our Ripon, CA travel plaza, provided below is a <br /> synopsis of testing that has been performed and the status of repair work that is to be completed. <br /> 3/8/05 and 3/9/05 -SB989 testing conducted by Dialysis North <br /> - Piping sumps 2, 5, and 6 failed; <br /> -Sumps need (3) 3 %2"x 2 W bulkhead fittings and (2)2 W x'/2" reducer test split boots with inserts <br /> (these <br /> will be going over the 2" pipe and %"copper line (siphon). <br /> -Contractor says these can be split because they are used for the test only and <br /> will be disconnected after the test. <br /> - Pictures were provided to the DFM (and subsequently to other contractors for bidding.) <br /> - Under Dispenser Containment(UDCs)failed on dispensers 13/14 and satellite 24 <br /> 4/28 Dialysis North contacted NTCA to say they were ready to begin repairs <br /> 4/29 Dialysis North contacted NTCA to say they declined to handle the repairs because of inability to use <br /> split penetration boots <br /> 4/29 NTCA contacted Flying J corporate to notify them of the status of events. NTCA began search for <br /> another contractor. <br /> 4/29-6/17 - NTCA contacted BSSR, Tait, CGRS, and MVP to see if they could repair and conduct the necessary <br /> retests. -----Contractors were given pictures and test results provided by Dialysis North. Dialysis <br /> North was also contacted to see if they would change their mind about making the repairs. <br /> - During this time frame, Flying J was updated regularly of the continued search for a contractor. <br /> - NTCA also contacted SJV to confirm that split penetration boots are not allowed. SJV noted that spli <br /> penetration boots are not UL listed and are therefore not allowed in their county even though they <br /> are widely accepted in other counties in California. <br /> -All contractors except MVP declined to bid after reviewing the results, pictures, <br /> and site. <br /> 6/20 -MVP performed a site visit and gathered information in order to prepare a bid for making the needed <br /> repairs <br /> 7/6 - MVP presented bid to NTCA <br /> 7/6 - NTCA submitted MVP bid to Flying J <br /> 7/7 - Flying J began contacting other contractors in order to get competitive costs <br /> 7/12 -Flying J notified NTCA they were seeking a more defined scope of work in order to get their own bids <br /> from other contractors <br /> 7/19 -Flying J received an itemized listing of equipment that needs to be repaired <br /> 7/19-27 - Flying J awaiting bid response from other contractors; the response from the other contractors has <br /> been very slow because they are swamped completing testing <br /> 7/27 -Flying J also requested timeline for completion of activities at this site <br /> 7/28/2005 <br />
