Facility failed to record all alarms (L9— 1129108, LIO—5117108, L10—
914107.) This is a violation of California code of regulation (CCR)section 2712
(b) Immediately start recording all alarms and how they are resolved. The
three instances listed are from records reflecting 300 days alarm reporting.
With respect to the first and second of the instances referred to on 1/29/08
and 5/17/08, the OMTEC alarm records enclosed clearly show that no alarms
automatically recorded on those dates. Therefore, the alarm records
maintained by the facility are entirely accurate and were appropriately
maintained by the facility's employees. No response action was taken
because there was nothing to which they needed to respond. The third, on
9/4/07,was misrecorded on 9/5/07, together with the response action taken.
Therefore, during the relevant 300 day period, one of the three noted
instances was a clerical error, and the other two were recorded accurately
and required no action. Flying J has reviewed with our employees the need
for absolute accuracy in recording alarms and the need to correctly note on
the form how the alarms were resolved.
2. 12. 89 and 91 leak detectors failed 3 gal/hr test at 10 psi.
