6/27/08
At about 11:30 am I went to 1501 Jack Tone Road, Ripon, (Flying J)to investigate the
status of the failed spill bucket. I met with Manager, Agnes, who informed that the spill
bucket is not repaired. She said Elite IV has been at the facility a couple of days ago to
take some measurements. I informed her that the service request(SR0054572)has been
approved and issued to Elite IV. She said she will call the contractor, Frank and Jones
(Norfolk, Virginia)to get an update. Later she informed me that Frank and Jones
informed her that the county has not issued the permit yet. I asked Agnes if they were
still filling diesel tank 1 and 2 (recorded as tank 4 and 5 in envisions). She said yes. I
asked for the inventory. She made a copy for me. A copy is attached.
Before I left I took pictures of all the dispensers in use(attached). Also reminded Agnes
that in the inspection report I had indicated that no fuel should be deposited in tank 1 and
2 until the repairs are made.
7/1/08
Went to the site and met with Agnes. She informed me that as of Friday(6/27/08) fuel
has not been deposited in tank 1 and 2. The fill bucket lid had a sign that said"Do not
drop fuel"(Picture attached). I requested for a copy of the inventory and she made me a
copy.
