4 <br /> The following is an itemized list of underground storage tank violations th etOptpbeen addressed for FLYING J TRAVEL PLAZA A618* as of March 29, 7. <br /> Open violations from May 04, 2016 inspection <br /> Violation #204 -Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. The monitoring plan uploaded into <br /> CERS for the onsite oil water separator is not current and/or not approved by the EHD. The monitoring plan for the <br /> onsite oil water separator indicates that the leak alarm triggers the automatic pump shut down and failure or <br /> disconnection of the sensor also triggers an automatic pump shut down. A copy of these plans shall be accessible <br /> on site at all times. Immediately make the necessary corrections and retain a copy on site. <br /> Violation #205 -Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. The monitoring plan for the onsite oil water <br /> separator indicates that the leak alarm triggers the automatic pump shut down and failure or disconnection of the <br /> sensor also triggers an automatic pump shut down. The monitoring plan must be uploaded to the California <br /> Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit <br /> for review by the EHD. <br /> Page 1 of 1 <br />
