The permit has <br />been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Vicki McCartney at (209) 468-9852 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Witness 24-hour hydrostatic (or equivalent) secondary containment testing on T-5 <br />premium unleaded STP sump and T-6 diesel (auto) STP sump. <br />4. Secondary containment testing for the T-5 premium unleaded STP sump and T-6 diesel <br />(auto) STP sump is required six months after installation. Provide 48 hours notification <br />to the EHD when this test is scheduled. <br />5. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />(may be issued to the installation company), or a Tank Tester's license issued to the <br />individual by the State Water Board, and <br />
