This work(with exception of work at Tank 4)was <br /> completed between December 2017 and April 2018, no EHD notification/permit was provided. The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at(209) 953-7699 to arrange the following appointments(a 48-hour notice is <br /> required for inspection appointments). Please be aware that inspections for emergency work must be <br /> scheduled within five days of permit approval. <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br /> and the Health &Safety Code. Monitoring system functionality and leak detector testing for Tanks <br /> 3 and 4 was verified during the monitoring system certification conducted on May 3, 2018 and <br /> May 4, 2018. <br /> Leak detector test:Leak detector on Tank 4 (87-octane)must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> 4. If concrete was broken near the Tank 2, Tank 3, and Tank 4 tank, a secondary containment test mus <br /> be performed on the Tank 2, Tank 3, and Tank 4 tank once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 1868 E.
