In accordance with the NoV and instructions from the inspector's office,the following actions have been <br /> performed. <br /> 1) Monitoring Plan not approved by EHD—CERS <br /> a. Monitoring plan has been submitted into CERS on 5/17/2018, per inspector's instructions.A copy of this <br /> plan has been passed to the site for on-site keeping. <br /> 2) Alarm Records Missing-4/13/18 record documenting L8 alarm. <br /> a. Please find attached work order 11110702 which details L8 alarm recorded on 4/13/18 and it's <br /> documented resolution. <br /> b. L8 System alarm referenced in this violation is in direct association with Jones Covey W.O 11110702.On <br /> site technician Jason Esqueda (ICC Service Tech 8817967 exp 8/12/19,Veeder Root TLS350 A23415 exp <br /> 4/19/19) performed wiring repairs on L8 on 4/13/2018, in doing so prior to performing the repair he <br /> removed L8 from programming in order to prevent L8 related alarms that could affect site operations. <br /> When the sensor was removed from programming during maintenance a System Setup Warning for that <br /> particular sensor was automatically generated by the Veeder Root. Once the repairs were completed <br /> and the sensor(L8)was reinitiated in the Veeder Root programing, the System Setup Warning self- <br /> cleared.After interviewing JCG tech,we have revised work order to incorporate self-cleared alarm. <br /> 3) Monitoring Plan—not made available during inspection <br /> a. Please find attached current monitoring plan, submitted into CERS 5/17/2018.A copy of this plan has <br /> been passed to the site for on-site keeping. <br /> b. Furthermore,account has been created within CERS that allows for local site operations to view, <br /> download, and print current monitoring plans and any CERS related requests in the future. <br /> 4) Leak Detectors replaced without a permit <br /> a. Please find attached approved permit acquired on 5/17/18 for leak detector replacements on <br /> 12/5/2017, 1/22/2018, and 4/7/2018. Please see service request number SR0079185. <br /> 5) Alarm Records Missing—L29 on 11/21/2017 <br /> a. Please find attached UST visual inspection checklist for 12/2017,amended by Designated Operator to <br /> account for the three L29 alarms on 11/21/2017. <br /> b. L29 Fuel Alarms referenced in this NoV were L29 Sensor Out Alarms, see attached tape.Sensor out <br /> alarms were created by Jones Covey tech Timothy Weiser(ICC Service Tech 8254370 exp 12/09/2018, <br /> Veeder Root TLS350 A32234 exp 06/09/2018) on W.O 9016214 on 11/21/2017 while making repairs to <br /> L7 Diesel Annular wiring. During process of repairs,technician inadvertently disconnected the L29 wire <br /> from its module,which automatically generated a Sensor Out Warning.Tech was able to reconnect wire, <br /> but only after three alarms had been generated. L29 Sensor Out Warning self-cleared upon <br /> 1 <br />
