Banner Street,Lodi CA <br /> L SUMMARY <br /> May 6,2003 <br /> Dennis Catanyag performed a routine underground storage tank (UST) inspection <br /> (Attachment 1) at the above referenced site. During the inspection, it was observed that a <br /> measuring stick was inserted in the diesel drop tube. New West Petroleum store manager, <br /> Jaylene Moore, was notified of the violation and the violation was also noted in the <br /> inspection report. (Attachment 1) Ms. Moore claims that the measuring stick did not belong <br /> to the facility and proceeded to show Mr. Catanyag the measuring stick that belonged to the <br /> facility. The measuring stick in the drop tube was removed in the presence of Mr. Catanyag <br /> on that day. (Attachment 2) <br /> July 23,2003 <br /> Dennis Catanyag was on site to verify the functionality of a replaced leak detector. While <br /> the technician from Elite IV, Robert Bush,was setting the calibration of the equipment, Mr. <br /> Catanyag checked the diesel drop tube to see if any item was obstructing the flapper valve <br /> of the overfill prevention system. This drop tube was clear of any such items. The 91-SUL <br /> and 87-RUL drop tubes were also checked and it was found that there were two measuring <br /> sticks in the 87-RUL drop tube. This issue was brought to the attention of the New West <br /> Assistant Manager, Michelle Edwards. Mr. Catanyag told Ms. Edwards that the sticks must <br /> be removed immediately. Ms. Edwards called the corporate office to get the required <br /> authorization to remove the sticks. Mr. Bush was willing to add this task into the original <br /> job order once he got the approval from Ms. Edwards. Both sticks were removed from the <br /> drop tube in the presence of Mr. Catanyag, one of which was marked with Williams Tank <br /> Lines. (Attachment 3) The nature of violation was written in a report. (Attachment 4) <br />
