Please see the <br />note below. Also, I know that this site was having a lot of alarms due to the ISD install. The alarms were not <br />shutting down the site, but seemed to be caused by an undetermined issue. We did send a construction crew out <br />and they determined the road noise might be a factor in the alarms. We looked at the wiring and could not find a <br />positive answer. Veeder-Root was contacted and they could not explain the problems either. I am not sure how <br />the problems were fixed at this point. 1 know this site uses other services companies sometimes. <br />I have attached copies of the Form C for return to compliance. Banner Street, Lodi, CA 95242. <br />I have listed each Violation in order with my response. <br />#2 — Walton Engineering sends out all test reports to each agency within 5 working days. Walton Engineering as <br />no record of performing a Secondary Containment test for this site on that date. The last SCT test performed for <br />this site by Walton Engineering was performed on 12-16-05. My records show that the next SCT is due on or <br />before 12-22-11. <br />#7 — Walton Engineering has no record of responding to the following alarms — L2:V-power stp sump on 10-14- <br />09,10-10-09, 9-22-09,9-16-09, 8-20-09, 7-30-09 and 6-25-09. Walton performed D.O. service on the following <br />dates — 6-25-09, 7-30-09, 8-20-09, 9-17-09, 10-15-09. On 9-23-09 Walton responded to a dispenser off line only. <br />Walton Engineering has no record of responding to the following alarms L5:87 annular— 8-21-09, 8-16-09, 7-30- <br />09, 7-27-09, 6-24-09, 6-21-09, 6-19-09. Walton Engineering has no record of responding to the following <br />alarms 1-1:87 stp sump — 8-17-09, 8-16-09, 8-14-09, 7-28-09, 7-27-09, 7-24-09, 6-24-09, 6-22-09, 6-21-09. All of <br />these dates are around the same time as our DO visits. <br />#12 , #13 and #21 Were taken care of during the annual test. <br />#23c — The Designated Operator did fail to list the following alarms listed in this section on line 5c of the DO <br />7/14/2010 <br />
