The Monitoring Plan on site are not current. In section V. continuous piping/ piping sumps and other <br />secondary containment does not list all the liquid detection sensor model found in the Fill Sumps. In <br />section VII. Periodic System Testing, this facility should be testing secondary containment components <br />every 36 months. In section X. Additional Information, please state that the flapper is set at 95% of tank <br />capacity for overfill protection. The Monitoring Plan must be current and approved by the EHD. <br />Complete and submit a copy of the Monitoring Plan for approval by the EHD by 05/09/11. An approved <br />copy will be returned for your records. <br />agajjms &� <br />214iquid was observed in the 89 STP sump. If water could enter into the secondary containment by <br />precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, <br />make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br />accordingly. Submit a statement how to ensure that the 89 STP sump will be maintained free of liquid. <br />- Corrected on-site. Technician removed liquid and took liquid as testing liquid. <br />Submit to SJC EHD within 30 days (05/09/11): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for the above violations <br />3. a revised Monitoring Plan <br />Notes- <br />- Last financial responsibility submitted 04/2010.
