The permit has been approved with the following conditions: 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable permits and requirements. Call Michelle Henry to arrange the following inspections (a 48 hour notice is required): Verification that the LD will detect a leak of 3 gallons per hour at 10 PSI. Note functionality was verified on May 12, 2009 during the Monitoring Certification. 2. Qualified service technicians must possess or work under the direct and personal supervision of an individual physically present at the work site who possesses all of the following: a. One of several permissible valid, current contractor's licenses issued by the Contractors State License Board (may be issued to the installation company), and b. A valid, current California UST Service Technician certificate issued by the International Code Council (ICC), and c. A valid, current certificate of training from the manufacturer(s) of the component(s) being tested, repaired, or serviced, and d. Each person installing or testing any component must hold a certificate of training from the manufacturer of that component. 3. Permit expires 180 days from date of approval. Note: If an extension to the expiration date is required, a written request must be submitted to this office prior to the expiration date. 4. All components must be certified by an independent testing organization and labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must bear the appropriate mark as required by the independent testing organization. 5. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing work. All new addenda supersede the previous ones. Page 1 of 2
