The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> ` <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Please beaware that inspections for <br /> emergency work must bescheduled within five days ofpermit approval. <br /> o. Functionality of the 91-octane STP relay must be verified to ensure that monitoring has been <br /> restored to meet the requirements of Title 23 California Code of Regulations and the Health & <br /> Safety Coda: verify that the above relay is fail safe (that it will shut down the turbine pump in <br /> the event of a power outage, or when the monitoring system fails or is disconnected). Note: If <br /> it is determined that a cold start is neoesaery, a copy of the alarm history aba|| be printed <br /> prior to performing the"cold start" of the Veeder Root system and made available at the time <br /> ofEHDinspection. <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of <br /> an individual physically present at the work site who possesses all of the following: <br /> a. One ofseveral permissible valid, current contractor's licenses issued by the Contractors <br /> State License Board (may be issued to the installation company), nraTank Tester's license <br /> issued to the individual by the State Water Boand, and <br /> b. Ava|id. current California UST.Service Technician certificate issued by the International Code <br /> Council (ICC), and <br /> o. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> banhed, repaired, or serviced. <br /> 6. Qualified installers must possess or work under the direct and personal supervision of an <br /> ' <br /> individual physically present at the work site who possesses all of the following: <br /> o.
