The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Thuy Tran at(209) 468-8257 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. 24-hour hydrostatic test (or equivalent) on UDC #3/4, 7/8, 9/10 and 11/12 sumps. <br /> 4. Secondary containment testing for the UDC# 3/4, 7/8, 9/10 and 11/12 sumps are <br /> required six months after installation. Provide 48 hours notification to the EHD when this <br /> test is scheduled. <br /> 5. A "UST Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owner's agent(CCR Section 2635(d)) before this upgrade is considered complete by <br /> EHD. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
