The designated operator employee training log failed to list the"Hire Date" for employee Desiree Burch. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This was corrected on site. This is a minor violation. 314 CCR 2631(a) Primary containment not tight(pre-Jul 2003). Weeping at the sheer valve and above the sheer valve was observed in dispensers 3/4 and 7/8, indicating a leak in the primary piping. All primary containment for the UST system must be tight. The service technician removed the product from the pipes and this appeared to have corrected the weeping of product. This was corrected on site. This is a minor violation. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 91 product line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed leak detector(LG 113-x listed, if applicable)and obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 91 product UST system may be red tagged to prevent fuel inputs. The 87 product line leak detector failed to detect a leak when tested. The service technician replaced the siphon jet and purged the lines and after four tries, the 87 product line leak detector was capable of detecting a 3-gallon per hour leak at 10 psi. The 87 product line leak detector was corrected on site. This is a repeat violation, Class Il. Overall Inspection Comments: On site to perform an annual monitoring system certification, spill container, and line leak detection inspection. Permission was given by Andrea Evans, manager, to do the inspection, take photos, and collect samples, if needed. The complete monitoring system certification inspection could not be completed today because there was a leak at the 87 flow meter at dispenser 11/12. Power out and sensor out were not tested today. Once the leak at dispenser 11/12 has been repaired, another inspection to complete the monitoring system certification will need Page 4 of 5
