No work orders were found for <br />the leak detector testing conducted on March 7, 2014. The testing was noted in the March 2014 designated <br />operator monthly inspection report, but no work orders were attached to the report. These records shall be <br />maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br />sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br />results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br />equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. The <br />missing work order was located by Ms. Evans, and a copy was placed on site at the time of inspection. This is a <br />minor violation. This was corrected on site. <br />308 <br />CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br />The 87 fill sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring <br />of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br />flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the <br />inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the <br />operator and contractor. This is a minor violation. This was corrected on site. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by March 25, 2015. <br />Consent to perform the inspection, take photos and collect samples was given by [NAME AND TITLE] . This <br />facility's designated operator is Elijah Pendergraft (ICC expiration date: 6/2/16) . The service technician was <br />Jarrod Cooke (ICC expiration date: 4/30/15 ), who had the following manufacturer's certifications: Veeder Root x. <br />5/6/16. <br />**The technician did not have a VMI certification available. Per Mr. Cooke, he recently took the test and has not <br />received his copy of the certification for the leak detector tester (LDT-5000). A copy of this certification current for <br />today's date must be submitted to the EHD. If this certification is not received, the leak detector testing will need <br />to be redone by a certified technician. <br />Note: The 91 spill bucket did not pass initial testing. The technician replaced the drain valve at the time of <br />inspection. The spill bucket was retested and passed. <br />Note: The cord on the 91 annular wraparound sensor was found to be too short, with no excess for removing the <br />sensor for testing. There were no records found, either in EHD files or in the facility work orders and repair logs, <br />indicating any work was done with this annular sensor since the last inspection on 3/3/14. Additional cording was <br />added at the time of inspection. <br />Note: The 87 tank form was left blank for "vent piping transition sump".
