These tests are required one every 12 <br />months. <br />o The Annual Monitoring Certification testing was three (3) days overdue from the anniversary <br />date of 2/27/13. The testing contractor originally scheduled the monitoring certification on <br />2/26/14 to this facility, however by accident, they switched the facility number with the other <br />station number when transferring the data, and that triggered the late test. Going forward, <br />Chevron will verify the schedule as well to ensure the monitoring certification is performed <br />within the timeframe. <br />• The designated operator failed to check and note the correct test dates on the November 2013 designated <br />operator monthly inspection report. The test dates section of the inspection report was left blank. <br />o The designated operator (D.0) corrected the D.O. monthly inspection report by entering the <br />correct test dates on the November 2013 D.O monthly report form. Please see attached DO <br />form. <br />• The 91 line leak detector (LLD) failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. <br />o On 3/7/14, the 91 LLD passed the 3 -gallon functionality test, witnessed by inspector Thuy <br />Tran. The follow up inspection report is attached. <br />o Please see attached Return to Compliance Certification. <br />Please contact me directly if you need additional information.
