Annual monitoring system certification, leak detector testing, and spill container <br />testing were last performed on February 27, 2013 and was completed on March 3, 2014, one month <br />past due. These tests are required once every 12 months. Take all necessary precautions to ensure <br />testing is performed in a timely manner. <br />204. The designated operator failed to check and note the correct test dates on the November 2013 <br />designated operator monthly inspection report. The test dates section of the inspection report was left <br />blank. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. REPEAT VIOLATION <br />311. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br />immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. <br />REPEAT VIOLATION <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by April 3, 2014 along <br />with a statement documenting the corrective actions that have been or will be taken for each violation <br />and any supporting paperwork. <br />Provided: Return to Compliance Certification form <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
