Financial responsibility documents have not been submitted to the Environmental Health Department <br />(EHD). Current financial responsibility documents are required to be submitted annually. Complete and <br />submit a copy of the financial responsibility by June 26, 2011. <br />5. The monitoring and response plans on site are not on the current forms. The monitoring and <br />response plans must be current and approved by the EHD. Complete and submit a copy of the <br />monitoring and response plans for approval by the EHD by June 26, 2011. An approved copy will be <br />returned for your records. <br />10 & 12. The 87 and 91 (split tank) annular sensor failed to activate an audible and visual alarm when <br />tested and, therefore, the tank is not monitored. The interstitial space shall be monitored with a <br />monitoring system connected to an audible and visual alarm system. Immediately contact a properly <br />licensed, trained, and certified contractor to replace the failed component with a functional component <br />(LG 113-x listed, if applicable) under permit and inspection of the EHD within one business day. <br />27. The dispenser sump liquid sensors and the 87 product turbine sump liquid sensor did not shut down <br />the 87 product turbine pump and the UST system does not meet the requirements for annual line <br />tightness testing exemption. Annual line tightness testing is required unless the UST system is <br />equipped with the following: <br />1. A continuous monitoring system that either activates an audible and visual alarm or stops the flow <br />of product at the dispenser when it detects a leak <br />2. A monitoring system that shuts down the pump or stops the flow of product at the dispenser when <br />it detects a leak <br />3. A monitoring system for piping other than that in UDC sumps (tank top sumps, transition vents, <br />etc.) is fail safe and shuts down pump when a leak is detected <br />Immediately schedule this test and provide 48 hours notification to the EHD or contact a properly <br />licensed, trained, and certified contractor to replace / repair the 87 turbine relay switch under permit and <br />inspection of the EHD. <br />Notes <br />1.
