Three existing diesel underground storage <br />tanks will be converted to gasoline tanks. The permit to remove the piping and other <br />appurtenances has been approved with the following conditions: <br />1. If the backfill material and soil is found to be contaminated it must be disposed of at an <br />approved disposal location. Treatment of soil onsite can only be performed under a hazardous <br />waste treatment permit. <br />2. A copy of the "Site Health & Safety Plan" must be on the jobsite for review: <br />3. See pages 6 & 7 of the permit application for required analytical tests and number of samples <br />to be taken and witnessed by this department. Use Form EH 23 046, Revised 07/22/10. <br />4. A copy of the fire department permit must be submitted at the time of removal. <br />5. The following information must be submitted within 30 days after the UST piping removal <br />date: <br />(a) Only analytical results that come directly from a third party California certified <br />laboratory with affixed chain of custody will be accepted by this office. <br />(b) UST Piping Tracking Record Sheet and/or UST Piping Hazardous Waste Manifest. <br />(c) Hazardous Waste Manifest for rinsate, residual fuel, or waste oil receipt. <br />6. This permit expires 180 days after date of approval. <br />7. The facility owner will be billed for additional EHD staff time expended beyond 3 hour <br />minimum permit payment per tank. <br />8.
