The designated operator failed to document all the alarms from the attached alarm history on the <br />1-23-14 and 7-24-13 designated operator monthly inspection reports and failed to document that they <br />were responded to appropriately. The missing alarms include: 12-20-13 L2 87 T1 annular low liquid <br />alarm and 7-20-13 L7 87 T-4 stp sump sensor fuel alarm. During the monthly inspection, the designated <br />operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br />response to any alarms to the monthly report. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 6-1-14. <br />Note: <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container <br />testing were last performed on 5-2-13 and are due before June 2014 . Testing was not completed <br />today. These tests are required once every 12 months. Immediately schedule these tests and provide <br />48 hours notification to the EHD.
