The permit has <br />been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. 24-hour hydrostatic test (or equivalent) on the T-3 (87) and T-5 (91) turbine sumps <br />and Dispenser 13 sump. <br />4. If concrete will be broken near the UST and/or piping, a secondary containment test <br />must be performed on the affected UST and/or piping once concrete is poured in <br />accordance with the manufacturer's guidelines. <br />5. Secondary containment testing for the T-3 (87) and T-5 (91) turbine sumps and <br />Dispenser 13 sump are required six months after installation of the secondary <br />components. Provide 48 hours notification to the EHD when this test is scheduled. <br />6. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
