The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Vicki McCartney at (209) 468-9852 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Witness 24 hour hydrostatic (or equivalent) secondary containment testing on T1 <br /> STP sump, T3 STP sump, T4 STP sump, T5 fill sump, UDC 7/8 (auto side). <br /> 4. Secondary containment testing for the T1 STP sump, T3 STP sump, T4 STP sump, T5 <br /> fill sump, UDC 7/8 (auto side) is required six months after installation. Provide 48 hours <br /> notification to the EHD when this test is scheduled. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service <br /> Technician certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> Page 1 of 2 <br />
