Please find below a list of the violations <br /> noted at the referenced facility and Pilot's responses. <br /> UST Program Inspection <br /> 1. Secondary containment testing was performed on 3-3-11 and a test report has not <br /> been submitted to the EHD. A copy of the test report must be submitted within 30 <br /> days of the test. Immediately submit a copy of the test report to the EHD. <br /> Jones Covey Group Inc. (Jones Covey) performed the testing on March 3, 2011 at <br /> this facility, and submitted a copy to the EHD and Pilot Flying J (PFJ) within 30 days. <br /> An electronic copy was also sent to the facility for your review at the time of <br /> inspection. Please find enclosed a copy of this testing as well. <br /> 2. Maintenance records for the L45 transition sump 1 high liquid alarm on 8/2/10 were <br /> not found on site. These records shall be kept on site for at least 3 years. <br /> Immediately locate and maintain the maintenance record for the L45 transition sump <br /> 1 high liquid alarm on 8/2/10 on site and submit copies to the EHD by 6/5/11. <br /> The L45 transition sump 1 high liquid alarm occurred while Jones Covey was onsite <br /> completing notified SB-989 testing for this location (see enclosed job ticket). While <br /> the alarm was logged by store personnel when it occurred, they were unable to <br /> locate a maintenance record during the inspection.
