A licensed contractor must be named with the following information provided: <br /> a. EHD contractor's and subcontractor's questionnaire (ignore if already on file) <br /> b. Certificate of worker's compensation insurance <br /> 2. A San Joaquin County EHD Fee sheet is needed. <br /> 3. Striker plates or drop-tube mounted bottom protectors are not illustrated or labeled <br /> below all accessible openings on the drawings provided. <br /> 4.
