In the event contamination is observed, confirmed or suspected as a <br />result of a; leaking UST system it is the responsibility of the owner or <br />operator to submit a work plan to EHD Site Mitigation Unit prior to <br />initiating any assessment or remediation activities. <br />b. TAinim= delays, a Corrective Action Plan maybe submitted <br />concurrent with tank closure or installation plans. Address one copy of <br />the Correc <br />�t ive Action Plan to the Site Mitigation Unit for review. For <br />further assistance contact the Site Mitigation Unit at (209) 46:3-3450. <br />PLAN REVIEW AND OPERATING PERMIT., FEES <br />1. New facility plan. review fee of $744.00 (8 hour minimum per facility), $500-00 <br />facility fee (includes one tank), $125.00 per tank after the first tank, Plus $10-00 <br />state surcharge fee per tank is due at time of plan submittal plus $17.50 per site <br />state surcharge. <br />2. New installation of at additional tank at an existing facility is $744.00 plan review <br />fee (8 hour minimumm per facility) plus $125.00 per new tank afterrf-irst tank, and <br />I <br />$10.00 per new tank state surcharge fee. <br />3. It should be -noted that a $744.00 plan review check fee required for each submittal <br />allows for up to eight, (8) hours of staffreview and/or inspection time. Additional <br />time spent on plan review, meetings, inspections, phone or office consultations will <br />be billed to the applicant at a rate of $93.00 per hour. <br />4. The maximum review time for UST installation plans is 40 working days.. A <br />preliminary review will be conducted by the EHD within 20 working days of <br />submittal date to identify any gross plan deficiencies. <br />5. if gross deficiencies are identified, the plans will be returned with a checklist <br />i <br />identifying plan deficiencies. <br />6. To expedite the review of installation plans, time and a half per hour overtime fee <br />maybe submitted to have review completed within 10 working days of submittal <br />date if staff is available to perform the review. <br />
