The "Training" section is not marked for <br />"Designated Operator Training". Please correct this to yes for all three tank monitoring plans. In the "Under <br />Dispenser Containment (UDC) Monitoring" section, "UDC Monitoring Stops Flow of Product to Dispenser" is marked <br />"yes". Please change to "no". The monitoring plan must be uploaded to the California Environmental Reporting <br />System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br />This is a Class 11 violation. <br />107 <br />CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />An accurate LIST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations <br />where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http-.//cers.calepa.ca.gov/ and upload a copy of the LIST Monitoring Site Plan. <br />This is a Class 11 violation. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the alarm history on the 10/31/2016 designated <br />operator monthly inspection report and failed to check that they were responded to appropriately. The missing <br />alarms include-. L9 fuel alarm in the vapor vent, recorded on 10/16/2016. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />This is a repeat violation, Class 11. <br />313 <br />HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. <br />Dispenser 7/8 shows signs of seepage in the center product pipe and one of the filters shows signs of slight <br />dripping. Dispenser 11/12 shows signs of light dripping on one of the filters. This indicates a leak in the primary <br />piping. All primary containment for the LIST system must be tight. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed component under permit and inspection of the EHD, if required. <br />This is a Class 11 violation. <br />316 <br />HSC 25290.1 (c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br />Liquid was found in the transition vent sump. Secondary containment shall be constructed to prevent any water <br />intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed about one <br />quarter cup of liquid from the transition vent sump and returned the sensor to its proper location. Designated <br />Operator records show history of water intrusion into this sump.
