It has been acceptable all this time by previous inspectors that if all info remains the same and current, in order to save materials, I just need to update with the new date. Now that you request the station to submit new CFR form every year I would like for you to clarify if it's a new requirement or just your preference. Violation 104. The date of application to install UST could be June 15, 2004 but the form says the "UST system" I would interpret the system would include pipes sumps sensors and if that the case then yes the system was installed in December 2005. But I will change them to June 15, 2004 to meet your requirement. I thought I changed them all to yes before I uploaded in CERS. OK I will do it again. There are still a couple of items that need to be addressed. Violation 103 - Your Certification of Financial Responsibility needs to be resubmitted with the signature and name of the witness or notary and the current date. See the attachment. (It would be preferred if you would submit this form each year instead of adding dates to an old 2011 form.) • Violation 104 - Our files have photos that indicate that the underground storage tanks were installed on June 15, 2004. Please revise the tank information in CERS to indicate the correct date when the tanks were installed. • Violation 106 — Please see the attachment. It shows that the Designated Operator Training is still marked "No". Please change this to "Yes" for all three tanks. When I reviewed the Monitoring System Certification reports submitted by Pinnacle Fuel Compliance Services, Inc. for testing completed on 3/5/15 and 3/7/16, 1 discovered that both reports are missing
