Eighth Street <br /> Stockton, CA. 95206 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to move dispenser 5/6 to replace dispenser 11/12 and cover Under Dispenser <br /> Containment Sump 5/6 on the underground storage tank (UST) system at the above noted address. <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District and <br /> local city building department for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cesar Ruvalcaba at (209) 953-6213 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that inspections <br /> for emergency work must be scheduled within five days of permit approval. <br /> a. Verification of the dispenser footprints contained by the under dispenser containment sump <br /> at (UDC) 11/12. <br /> b. Visual inspection of the piping within dispenser 11/12. <br /> c. 24-hour hydrostatic test (or equivalent) on UDC sump 11/12 and 5/6. <br /> d. Secondary piping test for the 87, 91 and diesel product lines: One hour pressure test per <br /> manufacturer's guidelines. If there are no manufacturer's guidelines, test shall be <br /> performed using an applicable method specified in an industry code or engineering <br /> standard. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. UDC's in which piping is capped and UDC is covered shall continue to be monitored and tested <br /> per UST regulations including but not limited to secondary containment testing and annual <br /> monitoring certification. The UST Tank Monitoring Plan and the UST Monitoring Site Plan in <br /> CERS shall be updated to reflect that dispenser 5/6 was removed but the UDC continues to be <br /> inspected by the Designated Operator, monitored and tested per UST regulations. <br /> 5. If concrete will be broken near the tank, a secondary containment test must be performed on <br /> the affected USTs once concrete is poured in accordance with the manufacturer's guidelines <br /> 1868 E.
