UM <br /> BRIM d <br /> , .... e ....... <br /> Item# Remarks <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the test dates for secondary containment, monitoring system <br /> certification, line leak detector and spill container testing on the January 2018 designated operator monthly <br /> inspection report. The designated operator did not include of the dates in the report. The designated operator also <br /> failed to document all the alarms from the attached alarm history on the October 2017 designated operator monthly <br /> inspection report and failed to check that they were responded to appropriately. The missing alarms include: L2 <br /> diesel annular low liquid alarm on October 4, 2017 and September 23, 2017.During the monthly inspection, the <br /> designated operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done and review the alarm history for the previous month, check that each alarm <br /> was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken <br /> in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br /> Liquid was found in the 87, 91 and diesel fill sumps. Secondary containment shall be constructed to prevent any <br /> water intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed <br /> approximately 3 cups of liquid from the sumps and returned the sensor to its proper location. <br /> This was corrected on site. <br /> This is a repeat violation, Class 11. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 91 octane line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor <br /> repair or replace the failed leak detector(LG 113-x listed, if applicable) and obtain a permit within one business day <br /> from the EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 91 octane UST <br /> system may be red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> This is an amended report and supersedes the original report issued on 03/08/2018. This report was generated <br /> on 04/09/2018. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, within 30 days of receiving this inspection report. . <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
