The designated operator failed to check and note the correct test dates for the monitoring <br />certification, leak detector, and spill container tests on the March 2012 through February 2013 <br />designated operator monthly inspection reports. These reports listed the tests as March 27, 2012. It <br />should have been listed as March 28, 2012. During the monthly inspection, the designated operator shall <br />check that all required testing and maintenance for the UST system have been completed and document <br />the dates they were done. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />204. The designated operator failed to verify that all employees have been trained during the January <br />30, 2012 monthly inspection. During the monthly inspection, the designated operator shall verify that all <br />facility employees have been trained. Ensure that designated operators performing monthly inspections <br />at this facility are including all of the required information on the reports. <br />303. The following sensors were at the bottom of the sump approximately two to six inches above the <br />lowest point of the sump, part way up the sump saddle, and not located to detect a leak at the earliest <br />opportunity: 87 STP Sump sensor 6 inches, 87 Fill Sump sensor 2 inches, Diesel STP Sump sensor 6 <br />inches, Diesel Fill Sump sensor 3 inches, 91 STP Sump sensor 6 inches, and 91 Fill Sump sensor 2 <br />inches. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. During the inspection, the technician relocated these sensors to the lowest point of the <br />sumps. Note: Owner states that previous inspector was ok with the location of the sensors in the straight <br />up position during the last monitoring system certification even though the lip of the sump prevented the <br />sensor from reaching the lowest point of the sump. <br />403. The STP and vent sumps did not have gaskets nor constructed to keep the UST system product <br />tight. Secondary containment shall be impervious to the liquid and vapor of the substance contained and <br />constructed to prevent structural weakening as a result of contact with any hazardous substances <br />released from the primary containment.
