The designated operator failed to check and note the correct test dates for the monitoring certification, leak detector, and spill container tests on the March 2012 through February 2013 designated operator monthly inspection reports. These reports listed the tests as March 27, 2012. It should have been listed as March 28, 2012. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 204. The designated operator failed to verify that all employees have been trained during the January 30, 2012 monthly inspection. During the monthly inspection, the designated operator shall verify that all facility employees have been trained. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 303. The following sensors were at the bottom of the sump approximately two to six inches above the lowest point of the sump, part way up the sump saddle, and not located to detect a leak at the earliest opportunity: 87 STP Sump sensor 6 inches, 87 Fill Sump sensor 2 inches, Diesel STP Sump sensor 6 inches, Diesel Fill Sump sensor 3 inches, 91 STP Sump sensor 6 inches, and 91 Fill Sump sensor 2 inches. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. During the inspection, the technician relocated these sensors to the lowest point of the sumps. Note: Owner states that previous inspector was ok with the location of the sensors in the straight up position during the last monitoring system certification even though the lip of the sump prevented the sensor from reaching the lowest point of the sump. 403. The STP and vent sumps did not have gaskets nor constructed to keep the UST system product tight. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD.
