S A N J D AR I N Environmental Health Department <br /> (L)j ;"� i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MICKE GROVE GOLF COURSE 1 11401 N MICKE GROVE RD, LODI September 30, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: In the maintenance shop eye wash station, a bottle of eye washing solution was found with an <br /> illegible expiration date.The year of expiration was noted to be 2017. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment,spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eye wash solution into proper working order by repair, replacement or testing. <br /> Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 30, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an <br /> appointment, please call (209)468-3420. <br /> Documents provided during inspection: Return to Compliance Certification form, Hazardous waste labeling <br /> information and hazardous waste label, Small Business Hazardous Waste Disposal Program brochure. <br /> Waste streams found: Used oil, shop rags, hazardous waste used oil filters, and lead acid batteries. <br /> Note: Used oil filters that are managed as hazardous waste per Jovito Garcia, were observed on top of a 5 <br /> gallon container. During the inspection, Jovito corrected issue onsite by placing the used oil filters into a 5 gallon <br /> covered container and affixed a hazardous waste label with all the required information.
