You should be aware that PHS/EHD may provide regulatory oversight for these sites
under an agreement which has been established between PHS/EHD and the Central Valley Regional
Water Quality Control Board.
PHS/EHD is responsible for maintaining and enforcing compliance with the laws and regulations
governing well construction, use and abandonment. In PHS/EHD correspondence dated March 30, 1993,
PHS/EHD referenced telephone conversations with your staff, who indicated that monitoring well, MW3,
would be destroyed. PHS/EHD requested that a work plan, permit application and proper fees be
submitted prior to destruction of this well. To date, this information has not been submitted.
Please be informed that the wells have not been monitored since their installation and if they are not
monitored and sampled they will be considered abandoned and as such would require destruction. If the
wells are not sampled by August 8, 1994, they will be considered abandoned/out of service and as such
require destruction.
If you have any questions, please contact Mary Meays of my staff at (209) 468-0337.
Donna Heran, REHS, Director
Environmental Health Division
Mary Meays, Senior REHS Diane M. on, REHS
Site Mitigation Unit Supervisor
MM\COSREDEV
cc: Dwane Milnes
A Division of San Joaquin County Health Care Services
