Time In: 3:10 pm <br /> r <br /> Time Out: 3:33 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: AVENUE GRILL,THE Date: 06/18/2020 <br /> Address: 506 W LODI AVE, LODI 95240 <br /> Owner/Operator: Telephone: (209)333-8006 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052091 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> FIRE AT FACILITY <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I received a call from Program Coordinator while conducting inspections <br /> When I arrived at the facility I met with facility operator who said that there was a smoke smell that was coming from the <br /> swamp cooler this morning. Per operator Lodi Fire Department responded and shut power off. <br /> I observed ceiling where ceiling tiles were removed. <br /> Per the operator the smoke may have been due to the swamp cooler motor. <br /> The power to the facility was not on when I arrived to the facility. <br /> The temperature of the 2 door reach-in under prep table is at 44F, <br /> walk-in is at 49F and the 1 door reach-in under prep table(between ice cream freezer and the wall) is 53F. <br /> All foods that are 50F and above and are potentially hazardous in these units shall not be given/sold to the Public. <br /> Provide a temperature of 41 F or lower of these units. <br /> During the inspection, at 3:28pm the electricity was restored, ok to operate. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Josh Abdon, Sig. not captured <br /> EH Specialist: STEPHANIE RAMIREZ Phone: (209)468-9851 <br /> FA0001133 C00052091 SCO04 06/18/2020 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
