SANJ O A Q U I N Environmental Health Department <br /> —00UNI Y <br /> Complaint Record Daily Time and Activity Summary from 6/5/2017 to <br /> 6/5/2020 for COON4AXUL located at <br /> MEDIAN STRIP NB 1-5 AT ELEVENTH ST <br /> OVERCROSSING <br /> Activity Date Recorded by <br /> 07/11/2019 Received a call and email from Juan Armendarez, PE 2546 ALANIZ <br /> 12 min <juan.armendarez@dot.ca.gov>, 209-948-3723, letting me know that the cleanup SC 544 <br /> was scheduled for July 17. <br /> 07/17/2019 Called Juan Armendarez, Caltrans Hazmat, 209-483-3088 at 8:00 a.m. asking for PE 2546 ALANIZ <br /> 126 min an update. Left a message. SC 544 <br /> Called Clean Harbors Supervisor, Scott, (707) 410-8242 at 9:08 a.m. asking for an <br /> update. He was not at the site but he said he would have the foreman, Joshua <br /> Jack,707-430-6732, give me a call when the cleanup was nearing completion. <br /> Received a call from Lauren Shelton, Caltrans Supervisor, 209-483-4466, <br /> informing me that he was at the site and would update me as the cleanup nears <br /> completion. <br /> Received a call from Lauren at 11:35 a.m. letting me know that the cleanup was <br /> nearing completion. Left for the site at 11:50 a.m. Met with Lauren and the <br /> foreman from Clean Harbor, Joshua Jack. The cleanup was done but the roll-off <br /> bins had not made it to the site yet. The waste was dug up and placed into three <br /> piles. No signs of diesel/oil contamination were observed in the areas that were <br /> remediated. Will need to drive by site again to make sure the waste has been <br /> taken. Left site and arrived to the EHD at 1:12 p.m. <br /> 07/29/2019 Received an email from Juan Armendarez, <juan.armendarez@dot.ca.gov> PE 2546 ALANIZ <br /> 48 min responding to Seargant Cynthia Segundo <CSegundo@chp.ca.gov> saying: Per SC 544 <br /> Hazmat Specialist Loren Shelton, the spill cleanup on 1-5 under the 11th St <br /> structure was completed on Wednesday and deemed clean by SJ County <br /> Environmental Health representative John Alaniz at 1300 hrs. The backfill was <br /> completed at 1500. Also, can you forward me a copy of the CHP Spill Report once <br /> it is completed? <br /> Attached were pictures taken by Loren Shelton showing the cleanup was <br /> completed. <br /> Discussed with Elena Manzo, Lead Senior ER Supervisor. It was decided that I <br /> did not need to go out to the site. <br /> Sent an email to the responsible party (RP), Julie Jimenez, at <br /> <jjimenez@ontracktransoffl.com> letting me know that I needed the final disposal <br /> records to close out the complaint. <br /> Received an email from Katy Hachman, Terminal Manager (RP) <br /> <khachman@ontracktransoffl.com>, with the prop 65 attached. I explained to her <br /> that was not the record I was looking for. <br /> Received the following update from Juan Armendarez <br /> <juan.armendarez@dot.ca.gov>: I just spoke to Scott from Clean Harbors. They <br /> received the profile today and the material is being classified as Hazardous and <br /> will be transported to the Buttonwillow disposal site. Clean Harbors will be picking <br /> up the bin before weeks end. Once it reaches the disposal site a final manifest will <br /> be sent to the trucking company, usually within 30 days. <br /> Page 3 of 5 <br />
