The following is an itemized list of hazardous waste violations that have not been <br /> addressed for CHASE CHEVROLET as of September 25, 2019. <br /> Open violations from September 13, 2019 inspection <br /> Violation#119-Consolidated manifest receipts failed to contain complete information. <br /> OBSERVATION: Consolidated manifest receipts found on-site for disposal of hazardous waste contained <br /> incomplete information. <br /> -Consolidated manifest for used oil dated 03-08-19 with Reference#79238303-1900729882 was lacking generator <br /> and transporter signatures. <br /> -Consolidated manifest for used oil with Reference#737830441702585570 was lacking dale and transporter <br /> signature. <br /> REGULATION GUIDANCE: (4)All copies of each receipt shall contain all of the following information: <br /> (A)The name, address,identification number,contact person, and telephone number of the generator,and the <br /> signature of the generator or the generator's representative. <br /> (B)The dale of the shipment. <br /> (C)The manifest number. <br /> (D)The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision(c), and its proper shipping description,including the hazardous class and United <br /> Nations/North America(UNMA)identification number, if applicable. <br /> (E)The name,address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported. <br /> (F)The transporter's name, address,and identification number. <br /> (G)The driver's signature. <br /> (H)A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> CORRECTIVE ACTION: Provide a corrective action statement and supporting documentation to the San Joaquin <br /> County Environmental Health Department within 30 days. <br /> lid This violation was corrected 13 This violation will be corrected by(date): <br /> 0 Supporting documents included l' 1 <br /> Describe actions taken or will be taken to correct violation: Spok-e- W <br /> t4! o si 1i ascnItL Jo-5 � LjOA Ae.f <br /> �r''nllu,41r'd� LAAOferI f0°14 ` Ma11it-e-K 4 ..nr� Arahi.feIt ` 6 p-�y <br /> �11fPr,T►1n 4, reQ k �1ntS ►�,o u�M jorw,%T- <br /> . <br /> Violation#120-Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Bills of lading for the recycling of lead acid batteries from 09-13-16 to 09-13-19 were not found on <br /> site. <br /> REGULATION GUIDANCE: <br /> (A)The generator shall use either the manifest or a bill of lading which fulfills the requirements of Title 13 CCR <br /> section 1161,to record the shipment of spent lead-acid storage batteries to a person who stores the batteries or <br /> who uses,reuses, recycles or reclaims the batteries or their components. <br /> (B)The generator shall retain, at the generator's place of business for at least three years, a legible copy of each <br /> manifest or bill of lading which identifies spent lead-acid storage batteries shipped to a person who stores the <br /> batteries or who uses, reuses, recycles or reclaims the batteries or their components. <br /> CORRECTIVE ACTION: Provide copies of battery core bill of lading records and a corrective action statement to <br /> the San Joaquin County Environmental Health Department within 30 days. <br /> Page 3 of 7 <br />
