Immediately take action <br /> to eliminate cockroach activity. <br /> Facility is temporarily closed until this item is corrected. <br /> The last pest control service performed by A-1 Exterminators was 6-7-20.A-1 service tech arrived on site this date at <br /> around 10am. <br /> Immediately treat area. Clean up all dead cockroaches, place new glue boards for monitoring, clean the floor, vents and <br /> door. Sanitize the area. <br /> Facility has multiple flies at the meat department.Take action to eliminate fly activity within the facility. <br /> Observed a dead rat or mouse on a glue board in the unused kitchen room.This room also has multiple dead cockroaches <br /> on glue boards and rodent droppings on top of the trash can and behind the trash can. The electrical room has rodent <br /> droppings on the floor. Immediately discard glue boards and replace with new ones. Clean up all dead insects and rodent <br /> droppings throughout the facility.
